Portfolio II Mishap

I have received straight “A”‘s for all my school work up until Portfolio II class (at University of Phoenix Masters of Science in Counseling program). I produced the same quality work in the Portfolio II class, yet, I was given a “Fail” grade. Huh, how does that work? I just want to know what is really going on?
